Professional Nursing Care

A case manager hospice nurse is responsible for assessing and managing patient care. In order to foster a trusting relationship and allow for continuity of care, only one case manager nurse is assigned to each patient. Nurses have a unique opportunity to be part of a patient’s remaining days, so they must be compassionate and empathetic.
